Plantar fasciitis

Plantar fasciitis (PF) is the most common cause of heel pain in adults. Although the exact cause is unknown, risk factors include middle age, obesity, excessive foot pronation, high instep foot, excessive running, flat feet and prolonged standing. The pathological process, often referred to as , ”heel spur" with severe pain syndrome, is caused by degenerative-dystrophic changes in the plantar fascia that occur at the heel bone attachment site. In a randomized study two methods of laser therapy were compared In the treatment of plantar fasciitis. Both methods, high-energy laser (HILT) and low energy laser (LLLT), showed significant improvement pain scale (VAS), treatment intensity index (HTI) and functional assessment questionnaire (FAOS) scores in both groups.

Low-energy laser LLLT is often used as a conservative treatment option for plantar fasciitis, but studies on its effectiveness have yielded conflicting results. Some studies suggest beneficial effects of LLLT on healing and pain reduction in, while others show no such effects. 

The HILT high-energy laser, on the other hand, uses high intensity laser light and can be more effective than LLLT, due to its greater penetration and stimulation of deeper tissues.

The biological mechanisms of action of laser therapy remain poorly understood. LLLT works by increasing microcirculation, stimulating nerve regeneration, immune processes and collagen production. HILT leads to tissue stimulation through ATP, RNA, DNA production and collagen growth.

The study suggests that both HILT and LLLT Are effective in the treatment of plantar fasciitis, by reducing pain, improving function and quality of life. HILT may have a greater therapeutic effect than LLLT.
